Returning Families

Submit your Letter of Intent to Return.

Complete one signed form for each student so OLSFMC can prepare class placement, staffing, schedules, and learning materials for the 2026–2027 school year.

  • State whether your student will return.
  • Confirm the student’s current level and expected grade.
  • Provide an electronic parent or guardian signature.

Attendance Notification

Report an absence or late arrival.

If your student will be absent or late, please notify the school as soon as possible by emailing the attendance office.

  • Include the student’s full name and OLSFMC level.
  • Include the date of the absence or late arrival.
  • For a late arrival, include the expected arrival time.
  • Provide a brief reason for the absence or delay.

Send a request to the school office.

Use this form to report an absence or late arrival, or to request grades and academic-progress information. Your submission will be sent directly to erudition4moorishchildren@gmail.com.

Do not enter passwords, banking information, Social Security numbers, or medical information in this form.
